ABSTRACT

Objective@#To investigate the longitudinal association of plasma Irisin concentrations with changes in blood pressure (BP) levels among children, and to assess the moderating effect of physical activity (PA) or sedentary behavior (SB) on the relationship between Irisin levels and BP.@*Methods@#Based on a cohort study, a cluster sampling method was used to select 3 651 school aged children from five schools in Guangzhou in 2017 at the baseline survey and follow up in 2019. Both at baseline and during follow up, PA and SB were assessed by validated questionnaires, and BP levels were measured by an electronic sphygmomanometer. A final sample of 521 children were enrolled based on the PA and SB at baseline. Plasma Irisin concentrations were measured by ELISA at baseline. Logistic regression analysis was recruited for exploring the associations of plasma Irisin concentrations with changes in BP. Moderating effects of PA and SB on the relationship between Irisin concentrations and BP were estimated using stratified analysis.@*Results@#Logistic regression analysis indicated that there was no significant association between Irisin concentrations and changes in BP levels among children ( OR =0.98, P >0.05). After stratification for SB, Irisin levels in the low SB subgroup were inversely associated with changes in diastolic blood pressure ( OR=0.87, 95%CI=0.77-0.98, P =0.02). In addition, SB level had a moderating effect on the relationship between Irisin levels and the DBP changes ( P =0.01).@*Conclusion@#Increased Irisin concentration is associated with the decrease of DBP level among low SB children. Furthermore, SB level shows moderating role in the relationship between Irisin concentrations and changes in DBP levels.

ABSTRACT

Objective:To compare the clinical effects of restrictive blood transfusion combined with hyperbaric oxygen preconditioning (HBOPC) and restrictive blood transfusion in the treatment of hip, knee arthroplasty (THA,THA).Methods:40 patients in the period of epidural anesthesia, femoral nerve hysteresis hip and knee arthroplasty were selected and randomly divided into two groups:restrictive transfusion group (maintain 80 g/L≤ Hb <100 g/L,n=20) and restrictive blood transfusion combined with HBOPC (HBOPC+maintain 80 g/L =Hb <100 g/L,n=20).The red blood cell transfusion, red blood cell transfusion rate, perioperative Hb,blood oxygen saturation (SO2),the incidence of hypotension during operation, hospitalization time and postoperative cerebral infarction, acute pulmonary embolism, pneumonia, myocardial infarction, wound infection rate and 90 days mortality rate were compared between two groups. Results:Compared with the restrictive transfusion group, the postoperative Hb,blood oxygen saturation (SO2) of restrictive blood transfusion combined with HBOPC group were significantly increased(P < 0.05);the red blood cell transfusion, red blood cell transfusion rate, incidence of pneumonia, wound infection rate were significantly decreased (P<0.05).Conclusion:Restrictive blood transfusion combined with hyperbaric oxygen preconditioning could improve the anoxic state of the hip, knee arthroplasty patients, which could effectively reduce red blood cell transfusion, reduce postoperative complications, has good clinical curative effect.

ABSTRACT

Objective To explore the expression of PD-L1 in peripheral blood mononuclear cells (PBMC) from patients with primary biliary cirrhosis (PBC) and its significance. Methods Peripheral blood of 23 healthy individuals and 23 patients with PBC diagnosed in Changhai Hospital was collected during February 2013 to December 2015.PBMC was separated by density gradient centrifugation, then cultured. The mRNA expression of PD-L1 in PBMC was examined by real-time fluorescent quantitative PCR (qRT-PCR),the expression of cytokines such as TNF-α,TGF-β,IL-23,IL-17 in medium was examined by enzyme-linked immunosorbent assay (ELISA).Two independent samples' t test was used to compare variables in two groups.Pearson correlative coefficient was used to reveal the relationship between two variables. Results mRNA expression of PD-L1 in experiment and control groups was 0.23±0.08 vs 1.27±0.40 (t=12.23,P<0.000 1) and there was statistical significance.Pearson correlation analysis revealed that PD-L1 was negatively correlated with IL-23(r=-0.531,P =0.009) with difference statistically. Conclusion PD-L1 might involve in the development of PBC,and could be the potential biomarker for predicting and treating this disease.

ABSTRACT

Objective To investigate peripheral blood T lymphocyte subsets and NK cells changes in the diffuse large B cell lymphoma (DLBCL) patients before and after chemotherapy, and analyse the relationship between the results and treatment. Methods Collect the 47 patients venous blood of the effective treatment DLBCL by pathology.T lymphocyte subsets and NK cells were determined by flow cytometric.Analyse the results statistically significant difference before treatment, the second chemotherapy cycle and the fourth chenmotherapy cycle compared with 50 healthy control persons. Results The levels of CD3+,CD4+,CD4+/CD8+ NK cells in DLBCL patients before chemotherapy [(70.04±8.87)%,(42.79±6.06)%,(1.68±0.59)%,(14.40±6.02)%]were lower than healthy controls [(63.89±6.67)%,(32.72±5.77)%,(0.85±0.25)%,(9.95±5.24)%](P < 0.05),and the level of CD8+ cells is higher than the healthy controls [(27.21 ±6.54)% vs.(39.92±7.11)%](P < 0.05).The levels of CD3+,CD4+,CD4+/CD8+ cells had significant difference between the second and the fourth chemotherapy cycle in DLBCL patients (P < 0.05).The levels of CD3+,CD4+,CD8+,CD4+/CD8+,NK cells had significant difference between the fourth chemotherapy cycle DLBCL patients and the DLBCL patients before chemotherapy (P < 0.05).Conclusion The DLBCL patients exist immunosuppression before chemotherapy.Peripheral T lymphocyte subsets and NK cells can be used as good reflect of the cellular immune function in DLBCL patients.Clinical parameters can be used for the immune function monitoring and providing guidance for the treatment options.
